Why medication safety is decided in the record

Medication errors rarely come from negligence; most often they come from information that is missing or lost along the way: an admission treatment copied wrongly, an allergy recorded in one unit and forgotten in the next. An EHR that carries the whole pathway, from the admission prescription to the ward prescription, makes the right information available at the right time.

Technology informs, people decide: the EHR’s alerts support the prescriber and the pharmacist; they do not make the decision for them.

AI reads the prescription the patient brings and extracts the treatment; the clinician checks and validates it. Scanning is built into the EHR: the data never passes through an external service.

When prescribing: LAP-certified

Drug interactions, contraindications, dosage alerts, compliance with good-practice guidelines: checked in real time by a module certified against the HAS framework.

During the stay: alerts that matter

Allergies are carried throughout the stay, and alerts are prioritised according to the patient’s context, so the prescriber is not buried in warnings.

End to end: traceability

Every prescription is traced and attributed, in the same record as the rest of the patient’s care, with clinical databases updated regularly.

LAP certification (prescribing decision-support software) follows a framework set by France’s Haute Autorité de santé, and is issued by an accredited certification body. It imposes functional and safety requirements on the prescribing module: prescription traceability, drug-interaction alerts checked against official clinical references, and regular updates of clinical databases. It complements the HDS and ISO 27001 certifications: where those protect the data, LAP certification covers the safety of care.
